The Cofo Provisions Wellness Program is a program for qualified wellness professionals and clubs.

Acceptance is by application, is not guaranteed and can be revoked at any time. If you are interested in applying, please fill out the application below.

To qualify, each wellness professional and club must offer in person or online events for their community and have members or clients. Your practice and/or program must also reflect the brand values of Cofo and be inclusive to all. 

Once you submit the form below, we'll respond in around 7 days. 

A few important things to know:

• If you have a practice or event/program/workshop you want us to partner with, we can offer information & product discounts as well as bulk products but not single-serve packets. 
• We ask that Cofo Provisions be the exclusive collagen sponsor at the event and/or for your practice. 
• Purchase of Cofo Provisions products at event or program pricing is for use at races, group activities and charitable or promotional efforts. We do not accept returns nor Cofo collagen resold.
